Testicular Torsion

Testicular torsion is a medical emergency characterized by the twisting of the spermatic cord, which cuts off the blood supply to the testicle. This condition requires immediate medical attention to prevent irreversible damage to the testicle and preserve fertility. Understanding the symptoms, causes, diagnosis, and treatment options for testicular torsion is crucial for individuals experiencing this condition and for healthcare providers managing it.

Understanding Testicular Torsion:

Testicular torsion is the twisting of the spermatic cord, which contains blood vessels that supply oxygen-rich blood to the testicle. This twisting can obstruct blood flow, leading to ischemia (lack of blood supply) and potential tissue damage within the testicle.

Causes and Risk Factors:

Testicular torsion may occur spontaneously or be triggered by physical activity, trauma, or anatomical factors such as:

  • Abnormal attachment of the testicle within the scrotum (bell clapper deformity)
  • Vigorous physical activity or trauma to the scrotum
  • Cold temperatures or sudden changes in temperature
  • Previous history of testicular torsion or family history of the condition

Common Symptoms:

Symptoms of testicular torsion may include:

  • Sudden, severe testicular pain
  • Swelling and tenderness of the affected testicle
  • Abdominal pain or nausea
  • Redness or discoloration of the scrotum
  • High-riding testicle (affected testicle may appear higher than the unaffected one)
  • Pain with palpation or movement of the testicle

Diagnosis and Evaluation:

Diagnosis of testicular torsion typically involves:

  • Medical History: A urologist will inquire about symptoms, onset of pain, medical history, previous episodes of testicular pain, and any recent trauma or physical activity.
  • Physical Examination: A physical examination of the scrotum and testicles will be performed to assess for swelling, tenderness, changes in testicular position, and other signs of torsion.
  • Diagnostic Tests:if available, Scrotal Doppler ultrasound may be ordered urgently to evaluate blood flow to the testicles and to confirm the diagnosis of testicular torsion.

Treatment and Management:

Treatment for testicular torsion is considered a medical emergency and may include:

  • Manual Detorsion: If diagnosed early, manual detorsion may be attempted by a healthcare provider to untwist the spermatic cord and restore blood flow to the testicle.
  • Surgical Intervention: Urgent Surgical exploration and fixation (orchidopexy) of the affected testicle and the other one too  may be necessary to prevent recurrent torsion and preserve testicular function. if delayed , it may lead to necrosis of testis resulting in excision after exploration .
  • Pain Management: Pain relievers and anti-inflammatory medications may be prescribed to alleviate discomfort and swelling associated with testicular torsion.

 Complications: Complications of untreated testicular torsion may include:

  • Testicular Necrosis: Prolonged ischemia and lack of blood flow can lead to tissue necrosis (death) within the affected testicle, resulting in permanent damage or loss of the testicle.
  • Infertility: Severe testicular damage or loss may impair sperm production and fertility in affected individuals.
  • Psychological Impact: Testicular torsion and its complications can have psychological effects, including anxiety, depression, or concerns about fertility and sexual function.

 Prevention: Preventive measures for testicular torsion may include:

  • Prompt Medical Attention: Seeking immediate medical attention for sudden, severe testicular pain or swelling can facilitate early diagnosis and treatment of testicular torsion.
  • Protective Gear: Using protective gear during sports or activities that pose a risk of testicular trauma can help prevent injuries that may trigger testicular torsion.
  • Regular Self-Examinations: Performing regular self-examinations of the testicles to detect any changes in size, shape, or texture can help identify abnormalities early and prompt medical evaluation if needed.
